Our Pledge to Your Information
Western Wyoming Medical wants you to know how we use and protect the important information you provide to us. Maintaining a record of the care and services you undergo at our center enables us to provide you with industry-leading care while also complying with specific legal requirements.
There may be occasions when Western Wyoming Medical will share your health information with members of our Organized Health Care Arrangement as allowed under HIPAA regulations and as necessary to carry out treatment, payment or health care operations. These members include patient care options affiliated with Western Wyoming Medical, and all medical staff, employees, volunteers, trainees, students and other personnel providing services as employed by our facility.
This notice details the ways in which we may use and disclose medical information about you, describes your rights and explains certain obligations we have regarding the use and disclosure of your medical information. All other uses and disclosures of your medical information may only occur with your permission, which you have a right to revoke at any time.
We are required by law to:
- Make sure that identifying medical information is kept private.
- Give you this notice of our legal duties and privacy practices with respect to your medical information.
- Follow the terms of the notice that is currently in effect.
How Western Wyoming Medical May Use and Disclose Your Medical Information
The following categories describe the different ways Western Wyoming Medical may use your health information within the clinic as well as how we will release your health information to persons outside the health system. Every use of information release is not listed here, but all permitted uses will fall within one of the following categories:
Treatment. Western Wyoming Medical may use your medical information to provide treatment or services. We may disclose your medical information to doctors, nurses, technicians, medical students or other clinic personnel who are involved in your care.
Western Wyoming Medical may share medical information about you to coordinate prescriptions, lab work and x-rays. Western Wyoming Medical may also disclose your medical information to individuals outside the facility who may be involved in your care.
Payment. Western Wyoming Medical may use and disclose your medical information to bill for the treatment and services you receive at our facility and to collect payments from an insurance company, a third party, or you.
Your information may also be used to review treatment and services received to assess the performance of our staff in caring for you. We may combine medical information about multiple Western Wyoming Medical patients to decide which additional services we should offer, which are not needed and whether certain new treatments are effective.
Information about your treatment or condition may be used to educate doctors, nurses, technicians, medical students and other clinic personnel, as well as with other healthcare providers to improve the care and services we offer. This same information is also shared to comply with laws and regulations or for accreditation purposes.
We may change this Privacy Statement from time to time. When we do, we will let you know by appropriate means such as by posting the revised policy on this page with a new “Last Updated” date. In some cases, you may be asked to agree again to our Privacy Statement or other terms, even if you have already agreed to accept them, because there were changes. Any changes to this Privacy Statement will become effective when posted unless indicated otherwise.
